The Limited Health Guarantee covers the cost of your dog only. We are not responsible for any associated charges, such as shipping or veterinarian fees, or mental stress.

To qualify for these Limited Guarantees, you must have your puppy examined by a licensed veterinarian of your choice within 72 hours (3 working days) of the date of purchase. (If shipped or paid in advance; the 72 hours starts at shipping or possession of the puppy at kennel)

HEALTH GUARANTEE

Mystery Mountain Pets (hereafter referred to as Seller) will replace a dog or refund the full purchase price of the dog (at Seller's option) if the dog should become ill from an immunizable disease within ten (10) days of the delivery of the dog.

Additionally, the Seller guarantees the dog against any debilitating genetic/congenital defects (a defect that would prohibit the animal from leading a normal pet life), for a period of 1 year from date of purchase. The dog, once again, to be replaced or purchase price refunded at Seller's option.

In order for the above provisions to be enforceable, a written statement from the purchaser's licensed veterinarian of choice must be furnished to Seller as to the exact cause of death (including an autopsy report) or specific defect.

In the event of a question regarding a defect, a second opinion from a licensed veterinarian of Seller's choice will be the governing opinion.

Seller DOES NOT GUARANTEE any dog against injuries or illnesses caused by the neglect or abuse of purchaser- (neglect or abuse also constitutes improper diet).

GENERAL TERMS & CONDITIONS

1. You must notify us the day of the initial examination to make a claim for a replacement dog.

an If you qualify for a replacement, you can choose a new dog of equal value if one is available.

b. If you choose a dog of greater value, you are responsible to pay the difference.

c. If we do not have a dog of equal value available within 90 days for your replacement, we will refund your dog's original purchase ($______) price.

d. Should the Purchaser choose to keep the puppy and accept refund, Seller will refund 50% ($_________) of the purchase price. Puppy must be spayed/neutered before refund check will be issued and Purchaser's veterinarian of choice must provide spay/neuter certificate to Seller.

2. We make no guarantee that your dog is show/breeding quality.

3. We are not responsible for vet fees, treatment costs, transportation costs, stress, or other expenses.

4. Purchaser and Seller agree that this Agreement will be governed by the laws of the State of Oklahoma and may not be altered, modified, or rescinded unless agreed upon in writing by Purchaser and Seller.

The undersigned agree to uphold all covenants of this Agreement as of this ___ day of ____________ 20___. By accepting the above described puppy, the Purchaser agrees to be bound by the provisions of this Limited Health Guarantee.
